Things you should know before scheduling an appointment:
Medication Management: Our practice offers medication management services to support your mental health journey.
Age and location: We provide treatment to clients aged 12 and older who are located in the state of Texas
Crisis Management: Please be aware that we do not provide crisis management services. In case of an emergency or a life-threatening situation, whether during our regular business hours or outside of them, please dial 911 immediately or visit the nearest emergency department.
Appointments are scheduled in advance, at a cadence we agree on, based on your goals, treatment needs, and our mutual availability. Payments for each appointment will be made through Alma or Headway by debit or credit card or ACH transfer.
We understand that unforeseen circumstances may arise, requiring changes to scheduled appointments. To ensure the efficient operation of our practice and accommodate the needs of all our clients, we have implemented the following cancellation policy:
Cancellations or rescheduling requests must be made at least 24 hours prior to the scheduled appointment time. This allows us ample time to offer the appointment slot to another client in need of services.
Cancellation or No-Show Fee: Patients who cancel an appointment or fail to attend with less than 24 hours’ notice are subject to a $100/$150 cancellation or no-show fee. Please be aware that you hold financial responsibility for appointments not rescheduled or canceled with appropriate advance notice. $100 Cancellation fee for established patients or $150 for new patients.
We understand that things come up that are out of our control. SAME DAY RESCHEDULES ARE ALLOWED AS LONG AS YOU SCHEDULE A NEW APPOINTMENT THAT DAY WITHIN THE NEXT 2 WEEKS. OTHERWISE, THIS WILL BE CONSIDERED A LATE CANCELLATION. No-shows, or clients who fail to attend their scheduled appointment without prior notification, will be charged the full session fee.
Exceptions to this policy may be made in extenuating circumstances, such as sudden illness or emergencies. We appreciate your understanding and cooperation in adhering to this policy to ensure the highest level of service for all our clients.
Prescription and Refill Policy
Our medication policy is designed with your health and safety as our utmost priority. We aim to provide you with the highest standard of care in managing your medications effectively.
Medication Management During Office Visits: To ensure the best possible care, we kindly request that requests for medication changes and prescription refills be handled during your scheduled office visits. Please make it a practice to bring your current medication list to each appointment. Typically, our provider will prescribe medications to cover your needs until your next scheduled visit. We strongly recommend scheduling your follow-up appointment before leaving your current visit to prevent any interruptions in your medication regimen.
Bridge Refills in Valid Cancellation Cases: In exceptional circumstances where you must cancel a scheduled appointment for a valid reason, our provider may offer a bridge refill as a one-time courtesy. While this may seem less convenient, our primary goal is to manage your medications optimally during office visits. This approach ensures that you receive the correct medication sat the appropriate dosage to maintain your health.
Frequency of Visits: The frequency of your visits will determine collaboratively you and your provider. It depends on factors such as the severity and complexity of your situation, current symptoms, the types of medications prescribed, and the availability of additional support from sources like family, friends, or therapists. Visits may range from monthly or more frequently initially, until you achieve stability and improved well-being, to quarterly visits once all necessary reports and assessments are up-to-date. In line with widely accepted community care standards, clients receiving psychotropic mediations must be evaluated by a medical professional at least once every 90 days to continue their medication regimen. Failure to attend an evaluation within this timeframe may require a follow-up visit to assess medication needs.
Managing Medication Supply: We strongly encourage you to keep track of your medication supply to avoid running out. We do not have the capacity to address emergent medication needs, so planning ahead is essential.
Requesting Medication Refills: When requesting medication refills, please ensure you have at least a three-day supply of medication on hand to prevent any disruptions. It’s advisable to check with your pharmacy first, as many refills can be authorized with a simple request to your pharmacist.
Your Rights Regarding Your Health Information
As a patient, you have the following rights regarding your healthcare information:
Right to Access: You have the right to access and obtain a copy of your healthcare information held by our practice, with certain limited exceptions. Requests for access should be made in writing to our Privacy Officer.
Right to Request Amendments: If you believe that your healthcare information is incorrect or incomplete, you have the right to request an amendment. Your request must be made in writing, providing a reason to support the amendment, and sent to our Privacy Officer.
Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures of your healthcare information made by our practice in the last six years. The list will not include disclosures made for treatment, payment, healthcare operations, or those authorized by you.
Right to Request Restrictions: You have the right to request restrictions on certain uses and disclosures of your healthcare information. We are not required to agree to your request but will make efforts to accommodate reasonable requests.
Right to Request Confidential Communications: You have the right to request how we communicate with you about your healthcare information, such as using a specific phone number or address.
Right to File a Complaint: If you believe your privacy rights have been violated, you have the right to file a complaint with our Privacy Officer or with the Secretary of the Department of Health and Human Services. Filing a complaint will not affect your care or treatment.
Protection of Substance Use Information: We understand the sensitive nature of substance use information and are committed to protecting the confidentiality of your substance use-related medical information. Federal laws and regulations, such as 42 CFR Part 2, provide additional protections for substance use disorder information. We will comply with these regulations and limit the disclosure of your substance use information, except as required by law or with your written consent.
Mandatory Reporting Obligations: As healthcare providers, we have mandatory reporting obligations in cases where we have reason to suspect child abuse, elder abuse, or threats of harm to yourself or others. In such situations, we are legally required to make reports to the appropriate authorities for further investigation or intervention, even without your consent or notification.

We reserve the right to modify this Notice of Privacy Practices at any time. If the terms of the Notice are changed, the new terms will apply to all your health information, whether created or received by Mentia Health and Wellness LLC before or after the date on which the Notice is changed. Any material changes will be promptly posted in our office and on our website, if applicable.
